    2) Fax a First Report of Injury to (580) 233-6037.

Triangle's Claims Line is available 24 hours a day, 7 days a week for your convenience.

Information you will need when reporting a claim may include:
     - Policy number
    - Client number
     - Injured employees: Name                            Marital status
                                    Address                         # of Dependants
                                    Telephone number         Date of hire
                                    Date of birth                   Length of time in current position
                                    SSN#                           Hourly or weekly wage
                                    Gender
     - Cause of accident
     - Type of injury
     - Date & time of accident
     - Medical provider or facility
     - Estimated return to work date
